You will receive a confirmation email or phone call for all events you sign up for. No charges will be incurred until confirmations are made.

I understand that my credit card will not be charged until the dinner / event is confirmed. If arrangements are made, and I must cancel within 24 hours of the Dinner Party, a $10.00 cancellation fee will be charged to my credit card. If I do not cancel, and I do not attend the Dinner Party, my card will be charged the full $20.00 dinner arrangement fee.

I will send a check to Eight for Dinner for $20.00 for the above chosen events. I understand that the check will not be deposited until the arrangements have been confirmed. If arrangements are not able to be made for the chosen events, I will be contacted and given the choice to hold the check for a future Dinner Party, or the check will be returned to me. If arrangements are made, and I must cancel within 24 hours of my Dinner Party, a $10.00 cancellation fee will be charged, and I will have a $10.00 credit with Eight for Dinner for a future Dinner Party. If I do not cancel, and I do not attend the Dinner Party, I will be charged the full $20.00 dinner arrangement fee.

** NOTE ** If your plans change after you complete this form, and you are no longer available for the week you requested, PLEASE call the office right away, so that we may fill your spot, and not disrupt the other seven members in your party. Thank you.
